Mental Health Evaluations Following Psychiatric Hospitalization
A previous psychiatric hospitalization can affect your ability to obtain or maintain a Firearm Owner’s Identification (FOID) Card in Illinois. In some circumstances, a mental health evaluation is required as part of the process of obtaining or restoring FOID eligibility.
Mosaic Minded Counseling provides FOID mental health assessments specifically for individuals whose FOID issue is related to a previous psychiatric hospitalization.
Our FOID assessment services are specifically intended for individuals who have experienced a psychiatric or mental health hospitalization and have subsequently had a FOID card denied or revoked, or have been advised that a mental health evaluation is necessary.
We do not provide FOID evaluations related solely to:
Criminal convictions
Domestic violence restrictions
Orders of protection
Developmental disability determinations
Other firearm prohibitions unrelated to psychiatric hospitalization

Who We Assess
What Is a FOID Mental Health Assessment?
Illinois has specific requirements for individuals whose FOID eligibility has been affected by admission to a mental health facility.
A FOID mental health assessment is an independent clinical evaluation of your mental health history, current functioning, relevant risk factors, and the circumstances surrounding your previous hospitalization.
The process and documentation required may differ depending on how long ago the hospitalization occurred.
Hospitalization More Than 5 Years Ago
If more than five years have passed since the applicable psychiatric hospitalization, Illinois State Police may require a mental health evaluation and completion of a Mental Health Certification for Firearm Possession by a qualified mental health professional.
As part of the evaluation, relevant mental health records and collateral information must be reviewed before a clinical determination can be made.
Hospitalization Within the Last 5 Years
If your psychiatric hospitalization occurred within the past five years, the process is different and may involve seeking relief through the Illinois FOID Card Review Board.
This process can require additional records, documentation, and a more comprehensive forensic mental health evaluation.
We will review your Illinois State Police documentation and the circumstances of your hospitalization to determine whether the evaluation you need falls within the services we provide.
